Job description

ESSENTIAL DUTIES & RESPONSIBILITIES FOR POSITION:
1. Assist patients with personal care needs such as bathing, elimination needs, transferring, dressing/
undressing, and ambulation together with any household task (e.g. making bed, changing
linens, laundry, light housekeeping, etc.) essential to patient’s health in accordance with the patient’s
individualized care plan.
2. Plan/prepare nutritious meals and assist in feeding patients, as directed.
3. Document services provided in accurate and timely manner in accordance with agency policy
and personal care plan. Maintain patient confidentiality at all times.
4. Measure and record patient’s temperature, pulse, respiration and blood pressure when requested
and/or directed by the Nursing Supervisor and patient care plan.
5. Report any changes in the patient’s condition or incidents to Nursing Department in a timely
manner.
6. Performs tasks, skills as outlined on care plan competently.
7. Notifies Nursing Department when clarification or education of a task is required.
8. Uses EVV system to clock in and clock out for each visit accurately and timely. If EVV is not available,
submits timecards/activity sheets in timely manner (i.e. weekly).
9. Implements appropriate infection control practices.
10. Demonstrates appropriate judgement skills to be able to make independent decisions in routine
patient care matters.
11. Assist patient with Passive Range of Motion (PROM) when requested and/or directed by the Nursing Supervisor/Physical Therapist or per plan of care

Special Requirements

75 Hour Home Health Aide approved Course
Prefer Spanish and English speaking
